Bus Travel Forms
Bus travel forms are required to be completed and returned within the first week of travel. It is important not only for the correct bus fare to be charged but also so we have emergency contact information should we need to get in touch for any reason.
By completing the below form this will assess for any available subsidies to ensure you are being charged the correct fare. below is the eligibility criteria for Free or partly subsidized travel.
Primary school students must live more than 3.2 Km from the nearest state primary school
Secondary school students must live more than 4.8 Km from the nearest state secondary school
Non-state school students must live more than the required distances above from both the nearest state and non-state school attended
Should you not meet the above distance criteria but hold a current Health Care Card (issued by Centrelink), Pensioner Concession Card (issued by Centrelink) or a DVA pensioner card with the students listed as dependent you will receive a subsidised fare rate.
